Eligibility Review

BEFORE YOU START - SAVE MONEY BY ANSWERING A FEW QUESTIONS!
Questions in the self assessment flow charts for Family Medicine Practice and Specialty Practice will help you decide if you should fill out an eligibility review form!

Eligibility review is NOT an application. It is a pre-screening process to determine if individuals are eligible to apply for a medical practice permit in Alberta. Only if the requirements of an eligibility review are met will an application form for licensure be issued.

The Eligibility Review process must be completed in a timely fashion. Inactive files will result in candidates having to submit a new Eligibility Review form and paying the applicable fee. A file is defined as inactive when there is no correspondence or activity:
     • Within any 6 month period after the College issues you an Eligibility Review reply
     • Within any 12 month period after the College receives your application form

Before beginning this process, please read the sections below for important information. 

  • Only the physician interested in applying for an Alberta Practice Permit can complete the online eligibility review form.
  • A charge of $210.00 (Cdn) is required after completion of the form in order to submit the material for review and assessment. 
  • The fee must be paid with a valid VISA, MasterCard, or American Express card and is non-refundable.
  • Allow 4-6 weeks for processing.
  • The College will issue a results letter based on information provided in the eligibility review form.
  • Only eligible candidates will be contacted by the College after the results letter is issued.
 

Reference Materials

Please reference the following information/materials when completing the online eligibility review form:

  • Canadian medical examinations completed (ie: LMCC, MCCEE, CCFP, CCFP (EM), FRCP(C), FRCS(C))
  • Citizenship status
  • Language proficiency testing information
  • Medical education history
  • Documents related to all postgraduate medical training
  • Certifications
  • Other qualifications
  • Practice history
  • Medical Identification Number for Canada (MINC) if previously assigned

 

Once completed, you will receive confirmation that the completed form and payment have been successfully submitted. You will be given an opportunity to print and/or save the submitted form. We recommend you keep a copy for your records.
