- Registration Fee: $300.00 (excludes GST)
- Annual Fee:
- Level II: $1,000.00 (excludes GST)
- Level III: $2,000.00 (excludes GST)
- Level IV: $6,000.00 (excludes GST)
- ALQEP Administration (for facilities performing Blood Gases only): $150.00 (excludes GST)
- ALQEP Programs (for facilities performing Blood Gases only): Registration & Payment for mandated programs is with program providers

- Pro-ration Policy (Annual Fee):
- For Facilities That Close During the Billing Year:
January 1 to March 31: refund 65% of the total fee (75% - 10% for administration fees)
After April 1: no refunds - For Facilities That Register During the Billing Year:
January 1 to September 30: invoice for the full amount
October 1 to December 31: invoice 35% of the total fee (25% plus 10% for administration fees)
